Once your primary care physician or healthcare specialist has made the referral to our practice, a staff member will call you to schedule your appointment and any radiographic imaging, if necessary. Please utilize this website for information regarding the practice, or just give us a call with your questions.

As soon as your appointment has been scheduled, please take the time to complete the new patient information prior to your appointment. If you would like some assistance in completing these forms, please arrive to your appointment 20 minutes early. We would be happy to help.

  • Appointment Cancellations: If it becomes necessary to cancel your appointment, please do so at least 24 hours prior to your scheduled appointment. Our office is very willing work with you to reschedule your appointment for the earliest availability.

  • Insurance: We are currently contracted with Blue Cross/Blue Shield, Veterans Administration and Medicare. We will gladly see Workers’ Comp and AHCCCS patients with a referral and/or prior authorization. As a service to our patients, we will file insurance claims to any insurance company. Patients are responsible for any unpaid balances. If your insurance requires a written referral from your referring physician, you must bring this with you at the time of your visit. If you forget to bring the referral, your visit may have to be rescheduled.

  • Copays: Co-pays are due at the time of service. If you cannot pay the day you are seen, prior arrangements must be made with our billing staff or practice administrator.

  • After your appointment: We hope that you will find your visit pleasant and helpful. We value our patients immensely. We are open to your suggestions in making your office visit a worthwhile experience. The information below may be of assistance with the progression of your treatment.

  • Testing: If the physician has ordered testing for you, our medical assistant will call you within three to five business days (depending on your insurance) to advise you of the testing date, and give you further instructions. Please do not hesitate to call if you have not heard within the stated timeframe.

  • Surgery: If the physician has recommended surgery, you will receive a phone call from our surgical scheduler. Our scheduler will coordinate your pre-operative testing, and prior authorize the surgery with your insurance. You will then be contacted with all the pertinent details and information.

  • Prescription Refills: Please contact your pharmacy first when you are in need of a refill. The pharmacy will notify us of your request. We require a 48 hour notice to process refill requests. Requests for written prescriptions may take up to 72 hours; we will call you when it is ready.

  • Medical Forms: Our office will complete medical forms on behalf of our patients at no charge. The patient’s portion of the form provided by the requesting party must be completed. Please provide an addressed envelope. Please note that forms requiring physician signature may take up to two weeks for completion.

  • Medical Records: All medical record requests must be submitted in writing in order to protect patient confidentiality. Please allow one week for processing your request.

  • Billing: We will file the charges for your office visit to your insurance company. Any amounts not paid by your insurance will be billed to you and are your responsibility to pay. Please feel free to discuss your bill with our billing staff or the office manager. We are very willing to work with you to resolve any questions or concerns you may have.
