Thank you for allowing Flagstaff Neurosurgery (FN) to care for your neurosurgical needs. It has been an honor serving Flagstaff and Northern Arizona for 21 years. I am writing to advise you of my retirement from my private practice, Flagstaff Neurosurgery, PLLC, effective September 30, 2023. I will remain available until that time for your neurosurgical and spine needs.
Since your condition may require future care, I suggest you arrange to place yourself under the care of another physician. If you need assistance in selecting another neurosurgeon or spine surgeon for your care, our FN staff would be happy to offer suggestions. However, please contact our office before September 30, 2023 if you require assistance. Additionally, your health insurer or primary care provider may be able to give you recommendations. I have contacted several of my colleagues in Northern Arizona regarding the closure of my private practice. Your healthcare is in good hands with them.
Your medical records are confidential; however, a copy can be transferred to a physician of your choosing. Included here, is a medical record request form that requires your signature and identification of the physician you have chosen. Please submit the form prior to September 30, 2023 by mail, email or fax. Once FN receives this form, a copy of your records will be transferred within 15 days.
